What this covers

This policy explains how Catherine Porter Orthodontist Limited handles information collected through this website. We follow the Privacy Act 2020.

If you are a patient

Your health information is handled separately and with more care than anything collected here. How we collect, use, store and share your clinical records is set out in our Patient Privacy Statement, which you receive at your first appointment and can ask for at any time.

What we collect here

  • Details you enter into an enquiry or booking form: your name, email address, phone number, and whatever you choose to write in your message

  • A mobile number, if you use the form to sign up for or update text appointment reminders

  • Standard technical information your browser sends, such as IP address, device type and the pages you visit

Please do not send detailed medical information through a website form. Anything clinical is better discussed by phone or in person, and we will tell you that if you do.

What we do with it

We use what you send us to reply to your enquiry, book or change an appointment, and send you appointment reminders if you have asked for them. If you go on to become a patient, these details become part of your patient record and are covered by our Patient Privacy Statement from that point.

We use website analytics to understand which pages people visit and how they find us. That information is aggregated and does not identify you.

We do not sell your information, we do not share it with advertisers, and we do not add you to a mailing list.

Cookies

This site uses cookies to make it work properly and to measure how it is used. You can block or delete cookies through your browser settings, though some parts of the site may not work as well if you do.

Where it is stored

Our website is hosted by Squarespace, and information you submit through a form is processed and stored on servers outside New Zealand. We take reasonable steps to satisfy ourselves that it is protected by comparable safeguards, as the Privacy Act requires.

Once we have dealt with your enquiry, we either transfer the details into your patient record or delete them.

Your rights

You can ask us for a copy of the information we hold about you, and you can ask us to correct anything that is wrong. We will respond within twenty working days. There is usually no charge.

If we decline to correct something, you can ask us to attach a statement recording the correction you sought, and we will keep it with the record.

Other websites

We link to other sites, including professional bodies and treatment providers. We are not responsible for how they handle your information and suggest you read their policies.

Complaints

If you are concerned about how we have handled your information, please tell us first and we will look into it. If you are not satisfied with our response, you can complain to the Office of the Privacy Commissioner on 0800 803 909 or at privacy.org.nz.

Changes

We update this policy from time to time and will publish any new version on this page with a revised date.
